REQUEST FOR PROPOSAL (RFP)

VIA RFP NO. 26-353 EMPLOYEE HEALTHCARE INSURANCE AND ADMINSTRATION SERVICES

Description Summary:

VIA Metropolitan Transit is seeking proposals from responsible firms to provide:

a) Third Party Administration (TPA) services for its self-funded VIAcare PPO plans.

b) Pharmacy Benefit Manager (PBM) services for its self-funded VIAcare PPO plan.

c) Fully-insured Medicare Advantage (MA) plan. d) Fully-insured Dental plans, and

e) Fully-insured Vision plans. The term of the Agreement will be for four (4) years with (1) one-year option to extend.
